	Re-enrolling families not changing their payment method will automatically be re-enrolled in SMART using their previous banking information.  Please submit any changes by May 1, 2008.	7/10/08 (first payment) 4/10/09 (final payment)
	Ten monthly payments beginning July 2008, final payment May 2009

**Plan 2, SMART annual application is due by May 1.  Please return SMART application form to the school office with a voided check if applicable. SMART charges an additional annual fee of $38 to use their payment service.
